Federal Motor Carrier Safety Regulations (FMCSR) require us to obtain a written authorization before pulling your motor vehicle record, contacting previous DOT-regulated employers, and running background screening. This page captures that authorization. Formal signed disclosures will be reviewed and re-signed in person before final hire.

Sign HereIn connection with your application for employment, Bulk Carrier Services Inc. (“BCS”) may obtain a consumer report about you, which may include information regarding your driving record (MVR), criminal history (where permitted by law), employment history, and other public records.
Pursuant to 49 CFR § 391.23, BCS may also contact previous DOT-regulated employers to investigate safety performance history including drug- and alcohol-testing records covering the past three (3) years, accident history, and reasons for separation.
You have the right to request a copy of any consumer report obtained about you and to dispute the accuracy of any information contained in it. Reports are prepared by consumer reporting agencies retained by BCS. The full FCRA “Summary of Your Rights” will be provided to you upon request and at the formal in-person signing.
By submitting this form you authorize BCS to obtain the consumer reports and information described above for the purpose of evaluating your application for employment.
